No, never. I'm not sure if it had surfaced when Regardie did his 1st Edition of "The Law Is For All" but it was not included in the 3-page fold-out of all the pages laid out (more or less) per Liber AL's instruction, that Regardie included in the book. Nor has it been included any where else, in either Hymaneaus Alpha's or Motta's editions, other than in miniature.
It is rather an odd "cover", too, with no sign of any planning, preparation or artistic design having gone into it at all, for all the world looking like some scrap of paper A.C. just happened to have picked up & then doodled away on, including maybe adding up the cost of some items for his weekly shop on it?
I particularly liked the - wine stain, is it? Only a round ‘ring’ mark could have set it off better, and perhaps an understated cigarette burn or two… In short, it could not have been more designed to look crap & have a less ‘professional’ finish if he had actively tried to do so, and in that regard alone it has a certain charm. In another life perhaps A.C. could have designed Fall album sleeves?

Fall album sleeves ...
Imagine that.
ROTFL!
